- Students are to fill out and sign an application for enrollment. A $25 enrollment fee must accompany the application. Fee is non-refundable.
- Each student is required to take a Basic English Reading Comprehension Test along with a Basic Math Skills Test. Students must pass both tests with a satisfactory grade in order to be eligible for enrollment.
- The total tuition payment for each class must be made prior to the starting date of class, unless payment arrangements are made in advance. Payment Plan is available.
- If the student will be paying tuition with a student loan, the student must supply MMSDT with proof of loan availability.
- A contract will be signed by both MMSDT and the student, expressing agreement with the school policies and procedures and tuition payments.
- Any student under the age of 18 years is required to have written permission by a parent
or guardian. The parent/gaurdian must sign the Enrollment Agreement along with the student.
Admission Of High School Seniors
High School Seniors are eligible to enroll in Mid-Michigan School of Dental Technology as long
as they obtain a letter of recommendation from the high school counselor. Any student
under the age of 18 years is required to have written permission by a parent or guardian. The
parent/guardian must sign the Enrollment Agreement along with the student.

- Cost per class: $800
- Experienced Students: Students can test out of classes/ $100 fee per class

- All tuition and fees paid by the applicant shall be refunded if the applicant is rejected by the school before enrollment. The application fee of $25 will be retained by the school even if applicant is denied admission.
- After signing a contract with MMSDT for enrollment, student has 3 business days following signature of the contract to cancel the contract with MMSDT if desired. Any and all tuition will be refunded within 30 business days of cancellation.

Refunds for cancellation of a class will be as follows:
- Classes cancelled before starting date will be refunded 100%.
- Classes cancelled within 2 days of starting date will be refunded 100%.
- Classes cancelled 3-6 days after starting date will be refunded 75%.
- Classes cancelled 7-14 days after starting date will be refunded 50%.
- Classes cancelled 15-28 days after starting date will be refunded 25%.
- After day 28 there will be no refund for cancelled classes.
