Admission Requirements

  • Students are to fill out and sign an application for enrollment. A $25 enrollment fee must accompany the application. Fee is non-refundable.
  • Each student is required to take a Basic English Reading Comprehension Test along with a Basic Math Skills Test. Students must pass both tests with a satisfactory grade in order to be eligible for enrollment.
  • The total tuition payment for each class must be made by the registration deadline for each term, unless payment arrangements are made in advance. Payment plan is available.
  • If the student will be paying tuition with a student loan, the student must supply MMSDT with proof of loan availability.
  • A contract will be signed by both MMSDT and the student, expressing agreement with the school policies and procedures and tuition payments.
  • Any student under the age of 18 years is required to have written permission by a parent or guardian. The parent/gaurdian must sign the Enrollment Agreement along with the student.

Admission Of High School Seniors

High School Seniors are eligible to enroll in Mid-Michigan School of Dental Technology as long as they obtain a letter of recommendation from the high school counselor. Any student under the age of 18 years is required to have written permission by a parent or guardian. The parent/guardian must sign the Enrollment Agreement along with the student.

Behavior of Students

  • Students are to respect their instructors authority at all times.
  • Students are to show respect to their fellow students.
  • Students are to use appropriate language. Excessive use of foul language may cause a student to be dropped from the class.
  • Students are to act with the highest academic integrity, honesty and truthfulness.
  • Cheating will not be tolerated.
  • Failure to abide by the behavior policies will be grounds for dismissal from the school without refund of tuition.

Appearance of Students

For the safety of all students, they shall dress according to this policy during hands-on classes:

  • Students with long hair must wear his/her hair back tightly or wear a hair net during all hands-on classes.
  • No loose clothing to be worn during all classes (especially sleeves).
  • Clothing and eye protection must be worn when instructor is casting.
  • Masks must be worn when using sand blaster and grinding models.
  • Protection for clothing, eye goggles and masks can be worn, at any other time, upon student or instructors request.

Class Attendance

Regular attendance is essential. All students are to be active participants in their class discussions and hands-on training. Students must come to class prepared, bringing the books and instruments needed. Active participation will be part of a student's overall grade. One clock-hour of lab time will be assigned to those who miss a class. Students are not to miss more than 2 classes per term (unexcused) for passing grade. It is recommended that students attend all classes since new skills are taught at each session. Make-up work is determined by the instructor. If make-up work is permitted, no additional fee will be charged.

Lab Attendance

The Lab is available for students to practice the skills they are learning in their hands-on classes. Starting Winter I of 2006 the Lab will be open Friday evenings 6-8 pm and Saturdays 9am-1pm. Lab times for Fall I, II will be announced the first day of class. Lab time will be required for partial credit for each hands-on class. For all hands-on classes, students must attend the Lab at least six (6) full clock-hours for full credit, although students are encouraged to attend for more hours, if possible. For missed classes (only 2 missed classes allowed) students will be required to attend the Lab at least one (1) full clock-hour per class missed.

Grade Requirements

Students are to complete all required assignments, participate in all class discussion, participate in all hands-on learning and complete all quizzes and tests to receive credit. In order to pass to the next course, students must pass the final (hands-on) with a satisfactory grade (2.0 or higher), showing that they understand all concepts and have acquired the skills taught.

In order for a student to pass to the next hands-on class, he/she must earn a 2.0 or higher in all courses.

Incomplete:

If there is an emergency that enables the student to complete his/her course of study, the student may ask for an incomplete grade. Student must receive approval from administration. Administrations approval will depend on the severity of the situation and the percentage of the course completed by the student. Student must have completed 75% or more of the course to be granted an incomplete grade. In order to change the grade from an Incomplete, the student must continue the course in the next term, finishing the weeks that he/she was enabling to complete. As long as the student finishes the course at the next available term, there will be no extra charge to the student. If the student does not finish at the next available term, additional tuition costs will be necessary.

Extra Credit

Extra Credit is considered nothing more than a good way to increase an unsatisfactory grade and does nothing in increasing a students understanding of the subject/skills. Extra Credit will not be allowed in any class, either online or hands-on. Students who have trouble understanding the content or developing skills have the option of asking the instructor for extra help. There will be no additional fees added for extra help.

On-Line Classes

On-line classes offer the student flexibility in learning the information not taught directly in the hand-on classes. However, this knowledge is essential for successful learning in the hands-on classes.

Students will have reading assignments weekly and discussion questions due weekly that must be posted to the discussion board by midnight of the due date given in the syllabus. Students will be required to read their classmates answers (5 to 7 days per week) and respond with useful comments that will help all students achieve better understanding of the material. The instructor will also offer comments on students' answers and will be available to help students better understand the subject.

The on-line classes function on email type postings, not a chat room atmosphere; therefor, there is no specific time each day that students will need to be online. Students are required to do their reading assignments, discussion question assignments, other assignments indicated in the syllabus and participate in discussion with classmates 5 out of 7 days per week by posting comments. The function of the on-line classes will be discussed in more depth with the student upon enrollment.

Refunds

  • All tuition and fees paid by the applicant shall be refunded if the applicant is rejected by the school before enrollment. The application fee of $25 will be retained by the school even if applicant is denied admission.
  • After signing a contract with MMSDT for enrollment, student has 3 business days following signature of the contract to cancel the contract with MMSDT if desired. Any and all tuition will be refunded within 30 business days of cancellation.

Refunds for cancellation of a class will be as follows:

  1. Classes cancelled before starting date will be refunded 100%.
  2. Classes cancelled within 2 days of starting date will be refunded 100%.
  3. Classes cancelled 3-6 days after starting date will be refunded 75%.
  4. Classes cancelled 7-14 days after starting date will be refunded 50%.
  5. Classes cancelled 15-28 days after starting date will be refunded 25%.
  6. After day 28 there will be no refund for cancelled classes.

Holidays

The following holidays will be observed by the school. The school will not be open on the following holidays:

  • Labor Day
  • Thanksgiving
  • Friday and Saturday after Thanksgiving
  • Christmas Eve
  • Christmas
  • New Year's Eve
  • New Year's Day
  • Good Friday
  • Easter
  • Memorial Day

Students wishing to miss class due to any other holiday must have it approved by administration two weeks prior to the date of the missed class.
